Scout Medical Forms

Good Evening To All. My name is Walter Kenderski (Nathaniel's Dad), I am in charge of collecting and managing Troop 84's Scout Medical Forms. Now that the troop is safely moving forward with in person meetings and weekend outings we need to have up to date documentation. We are asking that all Scouts participating in “in person” events have an updated A&B forms. At this time we are not allowed to host outings greater than 72 hours so form “C” is not necessary, but would be a good Idea to have completed at your next Doctor's appointment.  I apologize for sending this in a mass email but as of right now the majority of the scouts' forms are out of date. For those scouts on this message that attended summer camp, your forms are up to date. The scouts attending Monster Mash this weekend, I plan on being there sometime Friday early evening and you can turn your paperwork in then. For the rest I will be at the next In person meeting on November 2 to collect the remaining forms. I realize this is short notice and narrow  time frame. Please feel free to contact me with any issues.
