Medical Form Part "C"

Greetings Troop 84! As the Summer quickly approaches.I just wanted to send a reminder that if you are planning on participating in an event greater than 72 hours (ie. Summer Camp), Medical Form "C" will need to be completed and on file. This form requires a signature from your medical provider. So please print one out and take it to your annual physical or doctor appointment, this will save you a last minute visit to Urgent Care or The Minute Clinic.
